I am In-Network with: Aetna OHP-Trillium (There are many OHP plans, please be aware of which plan you are on.)
If you do not see your insurance co. listed above, I also provide out-of-network mental health services with any other insurance plan *if you have out-of-network benefits*. In fact, approximately 50% of my clients see me as an out of network provider. Please check with your insurance company, prior to contacting me, to determine if you have out of network benefits and what your out of pocket costs will be. I can not provide that information to you- only your insurance company can. Navigating insurance can be very confusing and frustrating, so here are some helpful questions to ask:
1) Do I have out of network benefits for mental health providers? 2) Is there an additional deductible for out of network providers separate from my in-network deductible. If so, how much? 3) If I see an out of network provider, is there a co-insurance (a percentage of the cost of the session fee) for those types of sessions? If so, how much? 3) How would I submit the claims? 4) How long would it take for the claims to be paid directly to me?
Billable services include the initial assessment and follow-up appointments. Services are billed different rates according to type of service provided, such as whether or not I am seeing an individual, couple, family or group. Other billable services include report writing, telephone conversations, attendance at meetings with other professionals you have authorized, preparation of records or treatment summaries, and the time spent performing any other service you may request of me. Some services will be covered by your insurance company, such as your intake and regular weekly/monthly appointments. Other services, such as no-show fees, letter and report writing, and collateral calls on your behalf are your responsibility.
Please contact your insurance company to determine your plan benefits. I will bill your insurance as a courtesy to you, or provide you with invoice to submit on your own in certain circumstances, but you are responsible for monitoring your coverage.
If you become involved in legal proceedings that require my participation, you will be expected to pay for my professional time even if I am called to testify by another party. [Because of the difficulty of legal involvement, I charge $250 per hour for preparation, time and travel for any legal proceeding.] Be advised that your insurer will not pay for these additional services.
